STENTING PROCEDURE AND ANGIOPLASTY IN THE TREATMENT OF CORONARY VASCULAR DISEASES

PROF. DR. HÜSEYİN OKUTAN

What is coronary heart disease? How is it treated? Diseases caused by problems blocking the coronary (feeding the heart) vessels are called 'coronary vascular disease'. A number of plaques called 'vein stiffness' among the public can block the flow of blood in these veins. With the inhibition of blood flow, chest pain occurs with rapid fatigue. Total obstruction of the vein causes a dangerous picture, which we call the 'heart attack', which is life threatening and can result in death. Is it possible to prevent this table? Obstruction of the vein can cause a life-threatening ‘heart attack, which can result in death. That's why; Every chest pain should be questioned considering the risk of coronary vascular diseases. The cause of chest pain seen especially by walking and climbing stairs should be investigated. In fact, anyone who has a sore chest should go to the doctor! Because if patients suffering from chest pain or having a heart attack apply to a healthcare institution without delay, it is possible to detect and open these veins instantly. In an other saying; Heart attack damage due to vascular occlusion can be prevented with the intervention performed in the first six hours for patients who have had a heart attack.

Just don't be late! How are we in terms of treatment in coronary heart disease? All kinds of treatment methods applied in the most developed countries of the world are carried out successfully in our country. In addition to treatment, precautions should be taken primarily to prevent disease. Negative factors such as bad eating habits, smoking, sedentary life should be changed. In some cases, medication is applied. The veins of the heart are displayed with the method we call coronary angiography. After determining the situation here, “Should Balloons be made? Should a stent be applied? Should surgery be done? ” it is necessary to focus on the answer to this question. How is the balloon and stent applied? Angioplasty called “balloon therapy” among the people is a method of widening the blocked vessels of the heart with a device. It has been successfully maintained since its first application in the world. This method of treatment is applied in Turkey about 15-16 years. In the first applications, as the veins opened after the balloon treatment were found to be clogged again, devices made of rusty steel mounted on the balloons, called stents, were started to be placed in the clogged vessel. If there is a multi-angle stenosis inside the vein, it can enter with the stent mounted on the balloon. If the vessel is too narrow and cannot be crossed with a stent, it is first entered with a balloon and the stent can be applied after the vessel is expanded a little. Stents are firmly mounted on the balloon. We use the balloon when inserting the stent into the vein. After mounting the stent, we lower the balloon and take it out of the vein. How is anesthesia applied while performing stent and balloon applications? General anesthesia is not required in both coronary angiography and stent and balloon application. We need to be in touch with patients while placing stents and balloons. We perform the patient by talking without sleeping. We numb the place we enter from the groin with local anesthesia. When does the patient return to his normal life? After angio, a 4-5 hour rest period is required in the hospital. After stenting or ballooning, patients should stay in the hospital one day. The patient can return to normal life one day after angiography, stent or balloon procedures. How is the patient followed after the treatment? Stent, balloon application or even bypass treatment is not a radical solution. Vascular stiffness continues after these treatments. During regular checks, the patient's medicines are reviewed, cholesterol is checked, whether there is chest pain, and a stress test is applied. If there is a problem in the effort test, angio can be reconsidered to display the vessels of the heart.
